Please arrive 10 minutes early to your first appointment to fill out the initial paperwork. Please bring an I.D. card, such as a driver’s license, and any insurance cards with you to your first appointment. Clients under the age of 18 must be accompanied by a parent or legal guardian at their initial visit.
